Unfortunately DTDs are inflexible wrt. to the order of
elements. MKVToolNix doesn't care, but that cannot be expressed via a
DTD.

examples: replace timecodes with timestamps
...
Part of an ongoing effort to replace the use of the term `timecode`
with `timestamp`. Timecodes have a very specific meaning in the
audio/video world, and it's not what MKVToolNix has been using the
term for.

Removed Subversion "$Id$" tags
...
Removed all lines with Subversion tags like "$Id$" because they are not
evaluated by Git. Collapsed consecutive empty lines into one empty line.
